I certify that the information and facts I provided on this application are true, accurate, and complete to the best of my knowledge and I understand that any deliberate falsifications, misrepresentations or omissions of fact on this application, my resume, or any other materials, or during any interviews, may result in my disqualification from consideration for employment, a withdrawal of an employment offer or termination from employment if I have been hired. 

I understand and agree that, if hired, my employment is at-will and can be terminated at any time, with or without notice, with or without cause and for any reason by either HMS Mfg. Co. or myself. I understand that the use of this application does not indicate that there are positions available nor does it create or imply an employment contract. This provision supersedes any oral or written representation to the contrary unless in writing and signed by both the President of HMS Mfg. Co. and myself. I also understand that no employment contract, ether express or implied for any period, is created hereby should HMS Mfg. Co. hire me. 

I authorize investigation of all statements contained in this application for any employment-related purpose. I release the listed references and all employers, except those specifically excepted, to provide you with any and all applicable information they may have. I hereby release all of these references and employers from all liability for any information they may give to HMS Mfg. Co. and waive any right that I might have to be provided with notice that they are releasing this information.

I agree that any action or suit against HMS Mfg. Co. arising out of my employment, application for employment, or termination of employment, including, but not limited to, claims arising under state or federal civil rights statutes, must be brought within the time period provided by the statute or within 180 days of the event giving rise to the claims, whichever is shorter or earlier, or be forever barred. I waive any limitations periods that are longer than 180 days.
